INTERESTED PERSON APPLICATION FORM

In the Law No. 6698 on the Protection of Personal Data ('Law'), personal data owners (hereinafter referred to as the 'Applicant'), who are defined as the person concerned, are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the Law. The applications to be made to Teamso Teknoloji Anonim Şirketi ('Company') regarding these rights must be submitted in writing or electronically and by the following methods in accordance with paragraph 1 of Article 13 of the Law and the relevant provisions of the Communiqué on the Procedures and Principles of Application to the Data Controller.

Application Methods:
In the applications to be made to our company within the scope of the Law on the Protection of Personal Data, in accordance with Article 5/2 of the Communiqué on the Procedures and Principles of Application to the Data Controller, which entered into force after being published in the Official Gazette dated 10.03.2018 and numbered 30356, it is obligatory to include the name, surname and signature if the application is in writing, T.R. identification number for citizens of the Republic of Turkey, nationality, passport number or identification number, if any, for foreigners, residential or workplace address for notification, e-mail address for notification, telephone and fax number and subject of request information.

1- Written Applications: Written applications to be made to the Company shall be made by delivering a wet signed copy of this 'Application Form', which contains the above information and is filled in completely, to the Company's Support Services Department in person or by proxy with a notarised power of attorney indicating that they are authorised to apply for the rights listed under Article 11 of the Law or through a notary public to 'Bostancı Mah. Bağdat Cad. A Block No: 492 Interior Door No: 1 Kadıköy / Istanbul' address through a notary public.

2-Electronic Applications: Electronic applications to be made to our company shall be made by submitting this 'Application Form' containing the above information and filled in completely.
- By signing with an electronic or mobile signature with a 'secure electronic signature' certificate defined in the Electronic Signature Law No. 5070, to the Registered Electronic Mail (KEP) address of our Company, or

- It should be sent to the Company's e-mail address [email protected] via the e-mail address belonging to the relevant person making the application and previously notified to our company by this relevant person.

Pursuant to paragraph 2 of Article 13 of the Law, we respond to the applications you have submitted to us as soon as possible and within thirty days at the latest from the date of receipt of your request, depending on the nature of the request. Our responses will be delivered to you in writing or electronically upon your request in accordance with the provision of the same article of the Law. This application form has been issued in order to determine your relationship with the Company, to determine your personal data processed by the Company, if any, and to respond to your application correctly and within the legal period. In accordance with the Law and within the scope of the Communiqué on the Procedures and Principles of Application to the Data Controller ('Communiqué'), the date of receipt of your application will be considered as the date of application. Your applications will be accepted by us as a result of our evaluations; If there is any missing information in your application, your request may be rejected by us. In order to eliminate the legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data, the Company reserves the right to request additional documents and information for identification and authorisation.

We will inform you about this matter and send you the information we request from you to complete your missing information. In the event that the information regarding your requests submitted within the scope of the form is not correct and up-to-date or an unauthorised application is made, the Company does not accept any liability for the requests arising from such incorrect information or unauthorised application.

As the Company, your information you submit with the application will be recorded with this form and your data will be deleted by us by taking the necessary technical and administrative measures when the destruction periods we have determined on the basis of the data category expire. If your application results require an additional cost in our scanning and report production process, the fees specified in Article 7 of the Communiqué may be allocated by you.
